​Please contact our office as soon as possible in regards to traveling with any animal. Each State/Country has its own requirements that continue to change.

When calling please provide us with the date the animal will be leaving, purpose of travel (pleasure/sale/auction), how many if more than one animal.

*NOTE* This process takes time, we must contact the states involved to confirm what the requirements are prior to setting up an appointment. to help spped up this peocess you may fill out the forms below and email/fax/text them in prior to your appointment.


WHAT IS A HEALTH CERTIFICATE?

A Certificate of Veterinary Inspection (CVI), also known as a health certificate, is used for movement from one state to another and can only be issued by an USDA-accredited veterinarian. Accredited veterinarians are responsible for confirming that their clients are moving animals in accordance with state and federal laws.
