When calling please provide us with the date the animal will be leaving, purpose of travel (pleasure/sale/auction), how many if more than one animal. This process takes time considering we need to contact other states to confirm what they require so we will call you to set up an appointment as soon as that information is obtained. Please fill out the forms below and email/fax them in prior to your appointment, this will help us in obtaining the information required.

WHAT IS A HEALTH CERTIFICATE?

A Certificate of Veterinary Inspection (CVI), also known as a health certificate, is used for movement from one state to another and can only be issued by an USDA-accredited veterinarian. Accredited veterinarians are responsible for confirming that their clients are moving animals in accordance with state and federal laws. To ensure you are following Michigan’s movement requirements, visit the animal movement pages.
